connecting levels

Linking the higher levels / altitudes with the effective GTD "routines" on lower levels (projects, actions) still is a problem for me. It seems this is in fact a problem with the standard system, as even hardcore GTDlers have to admit, that "the book" is not very helpful with this issue.
