LOFI Next Action Lists

I asked how people handle NAs in the GTD area, but I realized what I'm specifically looking for are paper-based ways of keeping lists. Do you keep them on individual sheets of paper in folders, or what?

I'm combining LOFI and records on my Mac, but I'm a paper planner user from way back and am kind of ingrained that way, plus I always have it with me.

Thanks.

The only thing it doesn't touch on is my file cabinet, which has everything in alpha order, just like The David says.
