Address book and PDA=>lo-fi

Ok, so after 8 years on Palm Pilots (I started with the Palm Professional, if that tells you anything), I'm going back to paper. The last round of non-Treos palm put out were crap, and there's nothing worse for GTD than a trusted system you don't trust. I've gone to hPDA for my tasks and such, and I have a calendar book, but I'm trying to figure out how to get my address book back down to paper in an efficient, expandable, and easy to carry way. I'd like to get it in a planner binder (probably 5.5 x 8.5in), and get it divided up by alphabet, with room to write in new stuff. I've been fiddling around with Mac Address Book and my old Palm Desktop, but the printing options are limited, and it does one continuous mess instead of breaking up by letter.

Any ideas? I also have access to Thunderbird and Outlook @work. Thanks!
