Wallet suggestions

Can any of you help me with my quest of a good GTD wallet:

It should not be complicated but I have not found anything yet, here are my specs:

1) 2 cash slots (one for cash, one for receipts for my expense account)

2) a few credit card slots

3) Index card holder, or notepad

4) small pen

Once I have this I will be set ... one of my biggest problems is not writing down things that I need to do when I am on the road and then forgetting them or stressing about forgetting them.

if you can help me I will be eternally grateful

After extensive hinting, I received...

After extensive hinting, I received the Levenger "international pocket briefcase" as a present for xmas.

Pros:
- Pretty sweet-looking. All of their stuff is good quality leather and has a spiffy executive look. Mine is red. It has an excellent "ooo, shiny" factor.
- Holds the right amount of stuff. I've got driver's license, three frequent-buyer cards, two credit cards and three insurance cards in there. There are enough pockets for things like receipts and one of those dollar-sized sheets of stamps. There are two pockets sized for 3x5 cards, which led me to the thing in the first place.
- Because it holds the right amount of stuff, this has replaced my regular wallet.
- Pen loop. (Lack of which being the big drawback, for me, to the classic hipster pda.)

Cons:
- A little too big to fit nicely in a front pocket, for those who carry their wallet so. It's a bit bigger than a HPDA.
- Pudgy when overstuffed. (Hopefully this will encourage me to move stuff OUT of it.)
